Enrolments 

Parents and carers wishing to apply for enrolment are required to complete an Application for Enrolment form (one form per child). This form allows you to formally express your interest in obtaining a placement at the school; however, submission of the form does not guarantee enrolment.

An Application for Enrolment is required for all students entering Kindergarten and Pre-Primary, including those already attending the school.

Please note that Dalkeith Primary School does not accept short-term enrolments.

The School Age Calculator can assist in establishing when your child is eligible to enrol.

Applications for Kindergarten Students

Required forms (click to open form and complete digitally):

Please submit the above forms together with the following supporting documents:

1. Birth certificate (if unavailable, contact the Registry of Births, Deaths and Marriages on 1300 305 021).
2. Australian Immunisation Register (AIR) Immunisation History Statement (dated within the last two months).
3. Proof of residency (e.g. lease agreement with a minimum of three months remaining, proof of property ownership such as rates notice or title deeds, or recent electricity or water bills).
4. Copies of Family Court or other court orders (if applicable).
5. Information relating to any health or medical condition, disability, or additional needs (if applicable).
6. If your child was not born in Australia, or if both parents were born overseas, please provide:
   1. Visa Grant Notice
   2. Passport
   3. Date of entry into Australia
   4. Australian Citizenship Certificate

You will be notified via email of the outcome of your application. If a placement is offered, you will be requested to complete an Enrolment Form Part B and a Healthcare Summary Form to finalise the enrolment.

In Term 4, families are invited to attend an orientation morning, where parents and children can become familiar with the teaching staff and Kindergarten environment. An Enrolment Information Pack will also be provided at this time.

Applications for Pre-Primary to Year 6 Students
